Job summary

Qualifications

• One year of experience working with disabled adults and/or frail elderly.
• Experience with medical and psychosocial problems of functionally impaired adults and the frail elderly.
• Exceptional communication and presentation skills relating to the frail elderly.
• Demonstrates case management skills and experience in the community health care delivery system.
• Computer literacy required.

Responsibilities

• Conducts comprehensive assessments of residents and ongoing re-assessments (psychosocial, physical, mental health, environment, spiritual).
• Writes comprehensive assessments and develops client-centered Community Living Plans.
• Identifies and arranges for community services based on Medicare/Medi-Cal understanding.
• Maintains care management relationships with clients and families; provides crisis intervention and advocacy.
• Documents all case management activity; maintains confidentiality and charting system.
• Participates in Continuous Quality Improvement and team meetings.
