Job summary

Institute on Aging Southern California, LLC is seeking a qualified social service professional to conduct comprehensive assessments, develop and implement a client-centered Community Living Plan, and coordinate services for disabled and older adults. The role includes home visits, coordination with healthcare providers, and ongoing case management to ensure timely, quality care.

Qualifications

• B.A or B.S in Social Work or related field required.
• One year working with disabled adults and/or older adults required.
• Experience with mental/behavioral health and substance abuse highly desired.
• Bilingual English/Spanish required.
• Exceptional communication and presentation skills relating to functionally impaired adults and their support systems.
• Demonstrates case management skills and experience in the community health care delivery system.
• Computer literacy required.

Responsibilities

• Conducts comprehensive assessments and ongoing reassessments of the client including psychosocial, physical and mental health, environmental and spiritual needs.
• Writes comprehensive assessments.
• Develops and initiates the Community Living Plan with the client, which is client-centered and consistent with guidelines.
• Conducts home visits and escorts clients to medical appointments as clinically indicated.
• Identifies and arranges appropriate community services based on Medicare, Medi-Cal and other programs.
• Maintains a care management relationship with clients and family as appropriate.
• Provides crisis intervention, advocacy, problem solving and therapeutic interventions.
• Maintains documentation within 24-48 hours and updates the care plan as needed.
• Operates within the National Association of Social Workers ethics and Community Living Fund policies.
