Job summary

The Institute on Aging Southern California, LLC in Merced, CA seeks a dedicated Case Manager/Social Worker to provide comprehensive assessment and ongoing care management for older adults and adults with disabilities. You will conduct home visits, coordinate services, and advocate for clients while maintaining confidentiality and ethical standards. Required: a B.A.

or B.S. in social work or related field, and experience with disabled/older adults. Bilingual English/Spanish preferred.

Qualifications

Bachelor's degree in Social Work or related field is required.
One year working with disabled adults and/or older adults preferred.
Excellent communication and presentation skills are essential for interacting with clients and teams.

Responsibilities

Conducts comprehensive assessments and ongoing reassessments of clients, including psychosocial, physical, and mental health needs.
Develops and initiates the Community Living Plan with client input.
Performs home visits and accompanies clients to medical appointments as clinically indicated.
Coordinates community services, monitors care plans, and maintains required documentation with timely progress notes.
