Blue Shield of California is seeking a Behavioral Health Care Management Clinician, Senior to support the Behavioral Health Triage Line and coordinate crisis intervention and care navigation. Required five years of relevant experience with behavioral health/triage, CA licensure, and advanced skills in assessment and de-escalation.

Qualifikationen

• CA license required as listed in the candidate criteria.
• Five years of relevant experience, including three in behavioral health or triage.
• Master's degree required for non RN-licensure, RN with assoc/BSN preferred.
• Certified Case Manager is preferred.
• Experience with psychosocial assessments and care planning.

Aufgaben

• Conduct initial clinical screenings and assess member needs and urgency.
• Provide crisis intervention and connect members with providers and services.
• Perform care navigation and referrals to appropriate programs.
• Review referrals and prioritize based on clinical appropriateness.
• Maintain a small caseload and support intake and routing activities.
