Job summary

PACE Southeast Michigan is seeking a Behavioral Health Specialist to join the Interdisciplinary Team and provide comprehensive behavioral health services to participants. You will conduct biopsychosocial assessments, crisis intervention, psychotherapy, and substance use treatment, while coordinating with Primary Care and Psychiatry to ensure integrated care.

The role requires an MSW and Michigan licensure, with experience in PCBH and DSM-5 knowledge.

Qualifications

• Master’s Social Worker from an accredited school of social work.
• Licensed to practice in Michigan; limited license may be considered.
• Experience in integrated Primary Care Behavioral Health (PCBH) model.
• Extensive experience with mental illness, serious mental illness, co-occurring substance use disorders, and primary health care problems.
• Knowledge of DSM-5 diagnostic criteria and advanced behavioral health concepts.
• Ability to plan, implement, and monitor patient-centered care.
• Knowledge of applicable laws and regulations.
• Cultural competence and ability to care for a diverse population.
• Ability to learn and use electronic health record systems.
• Proficiency in Microsoft Word, Excel, Outlook, and PowerPoint.
• Positive, flexible, solution-focused attitude with teamwork.

Responsibilities

• Provides behavioral health services to participants, including biopsychosocial assessment, urgent consultations and crisis intervention, psychotherapy, substance use treatment, and psychiatric hospitalizations.
• Follows up on recommendations and treatment plans to ensure adherence and outcomes.
• Educates participants on health correlates and engages in counseling.
• Maintains timely electronic health record documentation.
• Ensures quality, accessibility, and safety of behavioral health services.
• Collaborates with Primary Care Providers and serves as liaison between Primary Care and Psychiatry.
• Participates in treatment planning with the IDT and clarifies complex diagnoses.
• Assists with psychiatric hospitalizations and substance use treatment as needed.
• Provides supervision, staff development, and trainings in behavioral health topics.
• Contributes to clinical policy improvement and participates in committees.
