Responsibilities

• Gathers data, implements and evaluates care for an assigned group of patients based on orders and Nursing Care Plan.
• Takes and notes physician's orders accurately and consults with RN to plan care.
• Documents data and nursing process clearly and accurately in the record.
• Supports discharge planning and patient teaching, and makes appropriate referrals.
