Qualifications

• Has completed the prescribed curricula of a practical nursing program or equivalent coursework.
• Licensed Practical Nurse license (multi-state or Oklahoma).
• 0–6 months related experience.
• Knowledge of disease state management, bio-psychosocial model, and clinical pathways.
• Effective interpersonal, written and oral communication skills.

Responsibilities

• Gathers data, implements and evaluates care for an assigned group of patients based on physician's orders and Nursing Care Plan.
• Takes and notes physician's orders accurately; consults with Registered Nurse to plan patient care.
• Documents nursing process/care accurately and clearly.
• Addresses patient problems and progress toward outcomes; facilitates discharge planning and patient teaching.
