Job summary

Carrollton Springs seeks an Assessment Lead to oversee evaluation, admission decisions and care coordination for behavioral health patients in a full-time midshift. You will supervise staff, ensure timely documentation, and maintain EMTALA-compliant processes.

Applicants should hold a Master’s in Social Work or related field (or a Nursing degree with license), with at least two years of inpatient behavioral health assessment experience and required CPR/de-escalation certs.

Qualifications

• Master's degree in Social Work, Counseling or related field required.
• Bachelor's degree in Nursing required per state regulations.
• Independent Counseling or Social Work license or RN license required.
• Two years of inpatient behavioral health assessment experience.
• CPR and de-escalation certifications within 30 days of employment.

Responsibilities

• Facilitates response to service requests, including assessment and referral.
• Determines level of care and admission into services with follow-up.
• Performs initial evaluation and documents symptoms, needs, and plan.
• Demonstrates excellent customer service to patients and staff.
• Ensures proper pre-admission and admission paperwork and insurance processes.
• Maintains shift staffing and coaches staff on clinical decisions.
• Documents and reports issues to the director and completes calls/logging.
