Aufgaben

• Provide comprehensive medical management for patients in long-term care, ventilator, behavioral health, and subacute/post-acute settings.
• Conduct regular/daily rounding in assigned facilities to ensure continuity of care and timely follow-ups.
• Aggressively manage chronic and complex conditions with evidence-based interventions and respiratory therapies.
• Prevent avoidable hospitalizations through early intervention and vigilant clinical decision-making.
• Collaborate with facility staff, leadership, and physicians to optimize care plans.
• Discuss goals of care and palliative options with patients and families; support shared decision-making.
• Document thoroughly in the EMR and participate in interdisciplinary care meetings.
