Job summary

LSA Family Health Service in East Harlem seeks an Older Adult Services Coordinator to lead efforts for adults aged 60 and older. The role combines direct case management with staff training, program development, and outreach to strengthen aging services across LSA’s programs.

You will assess housing, benefits, and healthcare needs on home visits, document care in the database, train frontline staff, and build partnerships with aging providers to advance healthy aging in the community.

Qualifications

• MSW preferred; candidates with equivalent related credentials will be considered.
• At least four years of social work or case management experience, including connecting older adults to benefits and community resources.
• Professional fluency in English and Spanish is required.
• Experience serving low-income, immigrant, culturally diverse communities.
• Experience developing programs, training staff, and coordinating partnerships.
• Strong assessment, documentation, data tracking, organization, and communication skills.

Responsibilities

• Provide person-centered case management for older adults with complex needs, including benefits and housing coordination.
• Conduct home visits, outreach, service planning, follow-up, and timely documentation.
• Lead training and staff development on benefits and aging resources.
• Develop and maintain partnerships with aging services providers and community partners.
