Qualifications

• Must be able to provide a reliable personal vehicle to be used for business travel.

Responsibilities

• Conduct comprehensive initial assessments at the time of consumer admission into Older American Act funded programs.
• Perform ongoing assessment updates to reflect the consumer's status as required by program mandates.
• Provide case management services for assigned caseload, including face-to-face contact in home or community.
• Evaluate and plan services, monitor and review care, and coordinate services with providers.
