Job summary

Qualifications

• Knowledge of Trauma Informed Care Principles is essential.

Responsibilities

• Receive referrals and determine eligibility for the Memory Matters Program.
• Meet with potential participants to screen appropriateness for participation in the Program.
• Provide information to participant and caregiver related to the Memory Matters Program.
• Plan, facilitate and evaluate programming beneficial to individuals with Alzheimer’s disease and other dementias.
• Maintain a daily log of attendance and participation notes for each participant.
