Job summary

Catholic Charities, Diocese of Pittsburgh is seeking a compassionate Protective Services Investigator and Care Manager to help ensure older adults in Lawrence County are in the safest, least-restrictive living situations. You will investigate reports of elder abuse and coordinate care planning with families, providers, and the PA Department of Aging.

Qualifications

• Four years of experience in public or private social work, aging services, or related fields.
• Bachelor's degree or equivalent combination of experience and training.
• Excellent oral and written communication skills.

Responsibilities

• Obtains information and investigates complaints/reports of elder abuse, neglect, or exploitation.
• Performs home-based assessments/reassessments, investigations, and care management for participants.
• Develops care plans with participants/caregivers to promote the highest functioning in the least restrictive environment.
• Provides intensive care management and coordinating services to maintain medical, financial, and emotional stability.
• Coordinates with other agencies to fulfill service plan objectives for high-risk participants.
• Delivers community outreach and educational presentations on protective services.
