Job summary

The Lehigh County Area Agency on Aging, part of the Commonwealth of Pennsylvania, is hiring an Aging Care Manager 3 for the Protective Services Unit. This position provides protective services, performs case investigations, gathers information, and assists older adults and families to improve safety and independence.

Responsibilities include documentation, collaboration with professionals and community partners, and preparing court-related documents when needed.

Qualifications

• Minimum Experience and Training: One year as an Aging Care Manager 2 or Aging Program Assessor; or
• One and a half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

Responsibilities

• Case Investigation: Review and investigate referrals to determine risk level and service eligibility.
• Information Gathering: Obtain psychological and medical assessments and financial details from clients, families and professionals.
• Client Assistance: Guide older adults and families to resolve problems and improve circumstances.
• Professional Collaboration: Work with staff, agencies and community partners through home visits, conferences and meetings.
• Documentation: Maintain accurate records, reports, and entries in the standard system.
• Court Intervention Support: Prepare documentation for cases that may require legal action such as guardianship.
