The ideal candidate holds an active Michigan RN license, BLS, and a BSN (master's preferred) with at least 3 years of acute care experience.

Qualifications

• RN licensed in Michigan in good standing with BLS certification by orientation end.
• Minimum 3 years experience in an acute care hospital setting.
• BSN upon hire; master's degree strongly preferred.
• Ability to use word processing, spreadsheets, presentation programs and related software.

Responsibilities

• Move patients from admission through discharge with coordinated care.
• Act as advanced clinical resource to patients, families, and staff.
• Collaborate with interdisciplinary team for continuum of care.
• Monitor outcomes and participate in continuous improvement.
• Identify readmissions, communicate plan to team, and optimize transitions.
• Coordinate discharge planning and community resource referrals.
