Qualifications

• BLS certification by core orientation completion.
• Case Management Certification preferred.

Responsibilities

• Move patients from admission through discharge with coordinated care.
• Serve as an advanced clinical resource to patients, families and staff.
• Lead transition care planning and interdisciplinary rounds.
• Ensure LOS appropriate and coordinate post-discharge resources.
