Job summary

AltaMed Health Services seeks an Activities Coordinator to join the PACE interdisciplinary team in Compton. The role focuses on assessments, care planning, and implementing a diverse activity program tailored to participants’ physical, cognitive, social, and spiritual well-being.

You will use arts, crafts, music, movement, and community outings to create an engaging environment, with potential remote elements based on participant preferences.

Qualifications

• Minimum of a High School diploma or equivalent. A bachelor’s degree is highly desirable.
• Minimum of 1 year of experience in a social or recreational program for frail or elderly population (preferred). Experience with dementia care is highly desirable.
• Completion of 36 hours of Activity Leader course approved by the State of California within 30 days of hire.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• Experience in providing virtual activities is preferred.
• Valid BLS certification or higher per AHA or Red Cross guidelines.

Responsibilities

• Actively participate in care coordination activities, including Assessments and Care Planning.
• Design and oversee engaging activities for PACE participants across physical, cognitive, social and spiritual well-being.
• Utilize arts, crafts, animals, sports, games, dance, movement, music and community outings to engage participants.
• Adhere to PACE guidelines, policies and regulatory requirements under supervision.
