Job summary

The Activities Coordinator at AltaMed Health Services Corp. is part of the PACE IDT and designs and implements activities to promote physical, cognitive, social, and spiritual well‑being for participants.

The position uses arts, movement, games, and community outings to foster engagement and joy. You will lead a diverse activity program, coordinate schedules, and ensure compliance with program guidelines under the PACE Care Center Supervisor.

Qualifications

• Minimum of a High School diploma or equivalent. A bachelor's degree is highly desirable.
• Minimum of 1 year of experience in a social or recreational program providing and coordinating services for a frail or elderly population (preferred). Experience with dementia care is highly desirable.
• Completion of 36 hours of Activity Leader (previously "Activity Director") course, designed specifically for this position and approved by the State of California, is required within 30 days of hire.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• Experience in providing virtual activities is preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

• Actively participate in all care coordination activities, including Assessments and Care Planning.
• Design, implement, and oversee a variety of engaging activities tailored to participants' well‑being.
• Utilize arts, crafts, animals, sports, games, dance, music, and community outings in programming.
• Maintain planned schedules and ensure adherence to PACE program guidelines and policies.
• Coordinate with the PACE Care Center Supervisor and use remote technology when appropriate.
