Job summary

The role includes annual QI plan updates, data management, and education of program staff on quality improvement, with reporting and coordination of improvements from monitoring activities.

Qualifications

• Bachelor's or associate degree in nursing preferred.
• 2 years in healthcare quality improvement.
• One year experience working with the frail elderly population.
• Experience with Electronic Medical Record systems and MS Office suite.

Responsibilities

• Supports and assists with creation and maintenance of quality data sets.
• Oversees quality indicators and staff education on QI activities.
• Coordinates data collection, entry, analysis and reporting for QA initiatives.
• Facilitates CAPs and root cause analyses as needed.
• Attends quality committee and leadership team meetings.
