Innovative Integrated Health, Inc. is seeking an Activities professional to plan and implement therapeutic recreational activities for PACE participants, under the supervision of the Activities Manager.

The role emphasizes dementia-friendly programming, coordinating with IDT, and maintaining participant engagement and safety. Responsibilities include leading group activities, documenting participation, and ensuring supplies, equipment, and budget considerations are managed effectively.

Qualifications

• Knowledge of recreational therapy techniques for the elderly.
• Understanding of geriatric care needs and frailty considerations.
• Ability to design diverse activities including exercise, arts, events, and outings.
• Experience coordinating with volunteers and external vendors.
• Strong verbal and written communication to participants, families and staff.

Responsabilités

• Participate on the IDT and assist in assessments.
• Provide feedback on care plan outcomes to the IDT Manager.
• Coordinate group and individual activities addressing cognitive, physical, social and psychosocial needs.
• Develop activities including exercise, arts, celebrations, games and outings.
• Coordinate with transportation, nutrition, social services, nursing and rehab teams.
• Document daily activities and assessments accurately.
• Assist with consent forms when photos are requested.
• Plan dementia-specific activities and quiet-room programming.
• Support the Activities Manager to ensure compliance with duties.
• Maintain confidentiality of participant information.
