Job summary

Fallon Health seeks a Therapeutic Recreation professional to assess needs and develop individualized activity plans for Summit ElderCare participants. The role focuses on active ageing, mental health, and social connections alongside physical health improvements.

You will lead group and individual activities, support program planning, and collaborate with the interdisciplinary team to meet participant goals while maintaining documentation and compliance with PACE regulations.

Qualifications

• High school graduate or equivalent.
• CPR and Alzheimer's certifications, or willingness to be certified.
• One or more years of experience in an activities program in a health care setting serving a frail or elderly population within the last 5 years, or completion of Fallon Health offered training.

Responsibilities

• Conduct a variety of therapeutic recreational groups and individual activities to support cognitive, physical, social and psychosocial needs.
• Contribute activity ideas for monthly planning.
• Demonstrate programming for educational and culturally competent activities including exercise, arts and crafts, music, spirituality.
• Ensure participant choices and engagement in planning and conducting activities.
• Foster a warm social atmosphere and provide a supportive environment for participants.
• Document attendance and participation daily as assigned.
• Participate in Interdisciplinary Team meetings and case conferences as assigned.
