Job summary

CenClear seeks a diligent Mental Health Therapist to join the ACT team, delivering intensive, recovery-oriented services in homes and the community. The role collaborates with a multidisciplinary team to provide tailored treatment, records, and discharge planning, with occasional evenings and travel across counties.

The ACT program emphasizes integrated supports, housing, employment, and outpatient services, with a focus on delivering care 24/7 through the team’s coordinated efforts.

Qualifications

• Master’s degree or higher in a related field with practicum in health/human services
• Ability to work effectively as part of a multidisciplinary team
• Ability to provide services in community-based settings and clients’ homes
• Ability to work non-traditional hours as required by ACT program
• Reliable transportation and ability to travel between counties

Responsibilities

• Work 40 hours per week, 52 weeks a year, including some evenings
• Provide recovery-oriented, intensive, individualized treatment per agency best practices
• Promote a safe, welcoming therapeutic environment
• Collaborate with ACT team to address client needs
• Develop individualized, recovery-oriented treatment plans
• Coordinate referrals and exchange records with appropriate releases
• Maintain up-to-date medical records per regulations
• Complete required documentation in the EHR
• Attend trainings and supervision sessions
• Deliver services in offices, clients’ homes, and community locations
• Collaborate with multidisciplinary ACT team to meet treatment needs
