Qualifications

Employment Type: Full-Time/Hourly Non-exempt

Pay: $26.39 per hour

About the Role:

CenClear is seeking a dedicated and compassionate Mental Health Therapist to join our Assertive Community Treatment (ACT) team.

ACT is an evidence-based practice designed to provide highly individualized, comprehensive, and intensive services to individuals with severe mental illness.

This position provides an opportunity to make a meaningful difference by supporting individuals in their homes and communities while working collaboratively with a multidisciplinary team.

What You'll Do
  • Work 40 hours per week, 52 weeks a year (some evenings as needed)
  • Provide consistent, high-quality, recovery-oriented, intensive, individualized, and comprehensive treatment in line with agency philosophy and best practices.
  • Promote a physically and emotionally safe, welcoming, and therapeutic environment.
  • Work collaboratively with the ACT team to meet the individual needs of clients.
  • Coordinate referrals and exchange records as needed, with appropriate releases of information.
  • Develop individualized, recovery-oriented treatment plans that promote overall physical and behavioral health and wellbeing.
  • Collaborate with clients on goals for discharge and aftercare planning to support ongoing growth and change.
  • Maintain accurate and up-to-date medical records in accordance with state and program regulations.
  • Complete required documentation within the electronic health record (EHR).
  • Participate in required in-house and appropriate out-of-house trainings.
  • Attend staff meetings and participate in formal group supervision and formal individual supervision twice per month.
  • Provide services in a variety of settings, including offices, clients' homes, and community locations.
  • Work collaboratively with the multidisciplinary ACT team to address clients' treatment, rehabilitation, and support needs.
What We're Looking For
  • Master's degree or above from an accredited college or university in chemical dependency, psychology, social work, counseling, nursing with a clinical specialty in human services, or another related field that includes a practicum in a health or human service agency.
  • Ability to work effectively as part of a multidisciplinary team.
  • Ability to provide services in community-based settings and clients' homes.
  • Ability to work non-traditional hours as required by the ACT program.
  • Reliable transportation and ability to travel throughout Clearfield, Elk, and Jefferson Counties.
  • Must obtain PA State Police Criminal History, PA Child Abuse, FBI Criminal History, and Mandated Reporter Certificate before hire.

About the ACT Program

Assertive Community Treatment (ACT) provides highly individualized and comprehensive services based on each person's needs, which may include outpatient therapy, employment support, housing assistance, and other treatment, rehabilitation, and support services.

ACT provides individuals with the multidisciplinary support of a psychiatric unit while allowing them to receive services in their homes and communities. ACT team members bring expertise in areas such as psychiatry, social work, nursing, substance use, and vocational rehabilitation.

The ACT team provides services 24 hours a day, 7 days a week, 365 days a year, working together to meet the complex treatment, rehabilitation, and support needs of the individuals served.
