Job summary

CenClear is seeking a dedicated Mental Health Therapist to join its ACT team, delivering intensive, recovery-oriented care in clients’ homes and community settings. The role emphasizes collaboration with a multidisciplinary team to provide individualized treatment plans and ensure continuity of care.

The position requires a Master's degree in a related field, ability to travel across Clearfield, Elk, and Jefferson Counties, and willingness to work non-traditional hours as needed.

Qualifications

• Master's degree or above from an accredited college or university in a health or human service field with practicum.
• Ability to work effectively as part of a multidisciplinary team.
• Ability to work in community-based settings and clients' homes.
• Willingness to travel across counties as required by ACT program.

Responsibilities

• Work 40 hours per week, 52 weeks a year (some evenings as needed).
• Provide recovery-oriented, intensive, individualized treatment.
• Promote a safe, welcoming, and therapeutic environment.
• Collaborate with the ACT team to meet client needs.
• Coordinate referrals and exchange records with releases.
