Job summary

Sweetwater Care in Visalia, California is seeking an experienced LVN/LPN to provide nursing care within scope of practice, following physician orders and center policies.

The role requires Long Term Care experience, State LVN/LPN license, and BLS/CPR certification. You will chart using PointClickCare, monitor patient conditions, document changes, and collaborate with physicians during rounds to ensure quality patient care.

Qualifications

• Long Term Care experience is required.
• PointClickCare charting experience.
• Current LVN/LPN license in practice state.
• Current BLS/CPR certification or willingness to acquire.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follows established procedure for charting and incident reporting.
• Follows procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents changes in patient conditions including nutrition problems, infections, skin conditions or weight loss.
• Uses Quality Life Manual for identification, placement and monitoring of patients in programs.
• Notifies applicable departments of room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.

QUALIFICATIONS

• Current LVN/LPN license in practicing state.
• Current BLS/CPR certification or willingness to acquire one.
• Experience in nursing management is preferred but not required.

RESPONSIBILITIES

• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
