Job summary

Amergis Healthcare Staffing is seeking a Speech-Language Pathologist to work in a school setting on a travel assignment in Tualatin, OR. The clinician will perform student evaluations and provide and document therapy services per the plan of care and physician orders.

Active licensure as an SLP in the assignment state is required, with CCC-SLP preferred. Compliance with professional standards, CPR where applicable, and health-related screenings are expected.

Qualifications

• Active licensure as Speech-Language Pathologist in the state of assignment is required.
• CCC-SLP certification is preferred and may be required by state/contract.
• CPR certification if applicable.
• TB questionnaire/PPD or chest x-ray if applicable.
• Current health certificate as per contract or state regulation.
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• The SLP evaluates students and provides therapy services as outlined in the plan of care.
• Documents therapy progress and communicates with school teams and physicians as required.

The Speech-Language Pathologist in the school setting is responsible for performing student evaluations as well as providing and documenting therapy services in accordance with the plan of care developed for each individual student and the physician's orders.

Minimum Requirements

• Active Speech Language Pathologist (SLP) Licensure in the state of assignment required
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred and may be required by state/contract.
• Complies with all relevant professional standards of practice
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
