Job summary

Amergis, in Pataskala, OH, is seeking a Speech-Language Pathologist for school-based travel assignments. You will evaluate students and provide and document therapy services in accordance with each student's plan of care and physician orders.

Active SLP licensure in Ohio is required; CCC-SLP is preferred and may be required by contract. CPR, health certificates and compliance with standards are expected; must be 18+.

Qualifications

• Active SLP licensure in the state of assignment.
• CCC-SLP preferred and may be required by contract.
• CPR current if applicable.
• TB questionnaire or screening, chest X-ray if applicable.
• Health certificate current per contract or state regulation.
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Perform student evaluations in the school setting.
• Provide and document therapy services per plan of care.
• Follow physician's orders and maintain case documentation.

The Speech-Language Pathologist in the school setting is responsible for performing student evaluations as well as providing and documenting therapy services in accordance with the plan of care developed for each individual student and the physician's orders.

Minimum Requirements

• Active Speech Language Pathologist (SLP) Licensure in the state of assignment required
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred and may be required by state/contract.
• Complies with all relevant professional standards of practice
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
