Job summary

Amergis Healthcare Staffing is seeking a Speech-Language Pathologist for a school-based travel assignment in Salem, Oregon. The role involves evaluating students and delivering therapy services according to each plan of care and physician orders.

Qualified candidates hold an active SLP license in the state and CCC-SLP is preferred. CPR and health documentation may be required.

Qualifications

• Active Speech Language Pathologist licensure in the assignment state.
• CCC-SLP preferred and may be required by state/contract.
• Complies with professional standards of practice.
• Current CPR if applicable.
• TB questionnaire or chest x-ray if applicable.
• Current health certificate as required by contract or state.
• Must meet federal, state and local requirements.
• Must be at least 18 years old.

Responsibilities

• Perform student evaluations as part of school-based services.
• Provide and document therapy services per plan of care.
• Follow physician orders and program guidelines.

The Speech-Language Pathologist in the school setting is responsible for performing student evaluations as well as providing and documenting therapy services in accordance with the plan of care developed for each individual student and the physician's orders.

Minimum Requirements

• Active Speech Language Pathologist (SLP) Licensure in the state of assignment required
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred and may be required by state/contract.
• Complies with all relevant professional standards of practice
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
