Job summary

Provident Behavioral Health is seeking a School Social Worker to support students’ academic and social-emotional success across partner schools. You will assess, counsel, and case manage students, and collaborate with families, teachers, and administrators to meet student needs.

• Experience in school-based mental health and case management.
• Knowledge of IEP/504 processes and MTSS/RTI.
• Strong communication and collaboration abilities.

Responsibilities

• Conduct assessments, counseling, and case management for students.
• Participate in IEP/504 plans and multidisciplinary team meetings.
• Collaborate with teachers, administrators, and families to support student needs.
• Connect students with community resources and provide crisis intervention.
• Implement tiered interventions such as MTSS/RTI, PBIS, and SEL.
• Maintain clear, compliant documentation of services provided.
