Job summary

Middletown Public Schools is seeking a School Social Worker to support student academic success through social, emotional, and behavioral services at Lawrence Elementary School. You will liaise with families, school staff, and community providers to implement evidence-based interventions and ensure compliance with federal and state regulations.

The role requires a Master's in Social Work, LSW/LICSW certification, and strong knowledge of special education and crisis intervention.

Qualifications

• Master's Degree in Social Work required.
• LSW and/or LICSW certification required.
• Three years of relevant experience in social work or related services; school setting preferred.
• Valid State Driver's license required.

Responsibilities

• Conduct social-developmental assessments and record reviews to identify barriers to learning.
• Prepare reports for eligibility determinations, reevaluations and service planning.
• Participate in PPT, Section 504, and multidisciplinary meetings as assigned.
• Provide direct individual, group, and family-centered social work services.
• Develop, monitor, and evaluate intervention plans to improve attendance and behavior.
• Collaborate with teachers, administrators, and service providers to implement supports.
• Coordinate referrals to community agencies and mental health providers.
• Maintain confidential student records and required documentation.
• Travel between schools as required by assignment.
• Support data-informed decisions through data collection and analysis.
• Assist in mandated reporting and crisis response as needed.
