Job summary

DBHIDS in Philadelphia is seeking a Behavioral Health Liaison to support CBH Clinical Care Managers in the proper procurement of behavioral health services for CBH members and to monitor services to ensure delivery as authorized and best meeting clinical needs.

The role requires a Bachelor's degree in Social Work or related field, PA Child Abuse Clearance, and a minimum of two years’ direct service experience including assessment, counseling, and coordinating services.

Qualifications

• Bachelor's degree in Social Work or related field.
• PA Child Abuse Clearance required.
• Minimum of 2 years of direct service experience including assessment, counseling and coordinating services.
• Ability to travel within Philadelphia and surrounding counties.

Responsibilities

• Coordinate referrals including tracking, documenting referral status, notifying appropriate parties and following up with providers.
• Assist with referrals regarding ancillary and supplemental services for children, adolescents and adults.
• Coordinate information between CBH clinical management team and Clinical Care Management Specialist at DBH, Family Court and school-based programs.
• Provide CBH Members Services with the list of referred children, youth and families for follow-up and post-discharge contact to ensure adherence to the treatment plan.
• Facilitate and participate in interagency meetings.
