The HealthCare Connection, Inc. in Cincinnati, OH, is seeking a School Based Nurse Practitioner to join our dedicated Health team. This role delivers direct primary care within school-based health centers, collaborating with physicians and staff to provide high-quality, patient-centered care to student populations.
The ideal candidate holds a U.S. NP license, has at least five years in family medicine, and is comfortable working in a multi-disciplinary setting.
Formación
- Master’s or Doctoral degree in Nursing required.
- Board-certified Nurse Practitioner licensed in Ohio.
- Minimum 5 years of family medicine experience.
- Experience in a physician office preferred.
Responsabilidades
- Provide high-quality primary care to students and families.
- Assess, diagnose, treat, and monitor acute and chronic conditions.
- Obtain patient histories, perform exams, and order tests.
- Develop and manage treatment plans including medications.
- Promote preventive care and immunizations.
- Document encounters and maintain medical records accurately.
- Coordinate referrals and collaborate with healthcare team.
- Monitor progress and follow up on results for high-risk patients.
