Job summary

Responsibilities

• Patient Care- history/physical exams, diagnose, plan, order tests, and provide clinical consultation.

• Professional Practice- maintain professional behaviors, engage in quality improvement, lifelong learning, and mentoring.
• Care Management- promote self-management, family-centered care, coordinate referrals and treatments.
