Job summary

HealthPoint is seeking a Mental Health Therapist to deliver integrated mental and behavioral health care within a school-based health center. You will work with an integrated primary care team to assess and provide evidence-based interventions for behavioral health and substance use issues.

Responsibilities include managing a caseload, coordinating with medical colleagues, and documenting care in electronic records.

Qualifications

• Master’s degree in Social Work, Counseling, or related field is required.
• Current credential as a Licensed Social Worker or Licensed Social Worker Associate; Licensed Mental Health Counselor or Licensed Mental Health Counselor Associate; Licensed Marriage and Family Therapist or Licensed Marriage and Family Therapist Associate.

Responsibilities

• Deliver integrated mental and behavioral health care within a primary care team.
• Provide mental health and substance use assessment and intervention services.
• Maintain a caseload for ongoing mental health and substance use needs.
• Coordinate with outside referrals and specialty clinics as needed.
• Document services in patient records and MHITS system.
• Support program workflows and process improvements in clinics.
