Job summary

Primary Health Solutions seeks a School-Based Therapist to provide behavioral health services in the school setting, including assessment, counseling, and crisis intervention for children and adolescents.

The role requires a Master’s degree in a behavioral health field and current licensure (LSW or LPC preferred). Duties include developing treatment plans, collaboration with school staff and families, and maintaining HIPAA-compliant records in an EHR system.

Qualifications

• Master's degree in Social Work, Counseling, Marriage and Family Therapy, Psychology, or a related behavioral health discipline preferred.
• Experience providing therapy to children, adolescents, and families is preferred.
• School-based or community behavioral health experience strongly preferred.

Responsibilities

• Conduct behavioral health assessments and participate in diagnostic evaluations.
• Develop and maintain individualized treatment plans.
• Provide individual, family, and group therapy.
• Provide crisis intervention and safety planning.
• Collaborate with school staff, families, and community providers.
• Provide psychoeducation to students and families.
• Maintain accurate and timely documentation in the electronic health record.
• Complete treatment plan reviews, outcome measures, and discharge documentation.
• Participate in multidisciplinary meetings and case consultation.
• Meet productivity and quality expectations.
• Participate in supervision, training, and quality improvement.
• Comply with HIPAA, FERPA, Medicaid, and agency policies.
