Job summary

Primary Health Solutions is seeking a School-Based Therapist to provide behavioral health assessment, counseling, crisis intervention, and therapeutic services to students in the school setting. You will collaborate with students, families, school staff, healthcare providers, and community agencies to improve emotional well-being and academic success through evidence-based interventions.

Qualifications

• Master’s degree in Social Work, Counseling, Marriage and Family Therapy, Psychology or related behavioral health discipline preferred.
• Experience providing therapy to children, adolescents, and families is preferred.
• School-based or community behavioral health experience strongly preferred.

Responsibilities

• Conduct behavioral health assessments and participate in diagnostic evaluations as appropriate.
• Develop and maintain individualized treatment plans.
• Provide individual, family, and group therapy.
• Provide crisis intervention and safety planning.
• Collaborate with school staff, families, and community providers.
• Provide psychoeducation to students and families.
• Maintain accurate and timely documentation in the electronic health record.
• Complete treatment plan reviews, outcome measures, and discharge documentation.
• Participate in multidisciplinary meetings and case consultation.
• Meet productivity and quality expectations.
• Participate in supervision, training, and quality improvement.
• Comply with HIPAA, FERPA, Medicaid, and agency policies.
• Perform other duties as assigned.
