Job summary

Community Service Board Of Middle Georgia - Peo, Ltd. is seeking a qualified Apex CSI Case Manager for a school-based role in Dublin, GA. Travel to designated schools is required to deliver mental health services and coordinate resources for students and families.

The role emphasizes collaboration with parents, teachers and school counselors, along with rigorous documentation in EMR systems and adherence to timelines for progress notes and scheduling.

Qualifications

• High school diploma or GED required.
• Valid Georgia driver's license.
• Effective verbal and written communication skills.
• Strong interpersonal skills and ability to work with diverse communities.
• Ability to work independently and with others.
• Experience with Microsoft 365 Office products.
• Ability to organize, prioritize and meet deadlines.

Responsibilities

• Provide a broad range of social services to students and families at designated schools.
• Ensure clients attend medical appointments and take prescribed medications.
• Regularly contact families, teachers and counselors and document progress.
• Complete progress notes within 24 hours and ensure EMR entries within 7 days.
• Collaborate with schedulers to maintain monthly contacts.
• Communicate progress and adjust actions with staff to meet goals.
