Job summary

The Community Service Board of Middle Georgia is seeking a qualified Apex CSI Case Manager to deliver mental health services in designated school locations in Georgia. This school-based role requires travel to schools and collaboration with parents, teachers, and school counselors to coordinate care and resources for students.

Responsibilities include maintaining progress notes within 24 hours, updating EMR within 7 days, and ensuring regular weekly contacts and monthly schedules.

Qualifications

• High School Diploma/GED Equivalent required.
• Valid Georgia Driver’s License required.
• Effective verbal and written communication skills.
• Strong interpersonal skills and ability to work with diverse communities.
• Ability to work independently and with others.
• Experience with Microsoft 365 Office Products.
• Ability to organize, prioritize and meet deadlines.

Responsibilities

• Providing a broad range of social services to individuals served in Emanuel County and families.
• Ensuring individuals attend scheduled doctor appointments and take prescribed medications.
• Contacting consumers' parents, teachers, and school counselors weekly and reporting to supervisor.
• Completing progress notes within 24 hours and scanning EMR within 7 days.
• Working with scheduler to complete weekly schedule and ensure monthly contacts.
• Communicating progress toward goals and initiating corrective action when needed.
• All other duties as assigned.

LOCATION: Eastman, GA - Dodge, County, GA(THIS IS NOT A REMOTE POSITION)The Community Service Board of Middle Georgia's Child and Adolescent (C&A) program is seeking a qualified individual to serve as our next Apex CSI Case Manager. This school-based case manager will be responsible for handling cases related to delivery of mental health care to assigned clients at the designated school locations and will provide resources to parents and the school systems for individual patient needs. Please note, travel to and from schools is required.

Responsibilities of the Apex CSI Case Manager:

• Providing a broad range of social services to individuals served in Emanuel CountySchools and their families
• Ensuring individuals go to their scheduled doctor appointments and take medication as prescribed
• Contacts consumers parents, teachers, and school counselors on a weekly basis and reporting findings to supervisor and MD, ensuring all progress notes are correct
• Completing progress notes, no later than 24 hours after delivering the service, having said progress notes scanned and shown in the EMR no later than 7 days after providing the services, and checking EMR weekly to ensure that progress notes are showing in EMR
• Working with scheduler in completing weekly schedule to ensure monthly contacts are met and maintained
• Communicating regularly with staff on progress toward defined goals and/or required results, providing specific feedback, and initiating corrective action when defined goals and/or required results are not met
• All other duties as assigned
