Job summary

The Community Service Board of Middle Georgia is seeking a qualified PSR-I A Case Manager for the Eastman, Georgia area. The role involves adhering to treatment plans, providing services and interventions, and coordinating with a multidisciplinary team to ensure consumer progress.

Strong communication and organizational skills are essential. Minimum requirements include a High School Diploma/GED, valid Georgia driver's license, and experience with Microsoft 365.

Qualifications

• High School Diploma/GED or equivalent.
• Valid Georgia Driver’s License.
• Effective verbal and written communication skills.
• Strong interpersonal skills and the ability to work effectively with diverse communities.
• Ability to work independently and in collaboration with others.
• Experience with Microsoft 365 Office Products.
• Ability to organize, prioritize and meet deadlines.

Responsibilities

• Adhere to treatment plans and provide services and interventions as planned; write progress notes (24-48 hrs) and bill for services provided.
• Complete notes within agency policy timeline.
• Recognize consumer needs and assist them in obtaining appropriate services.
• Provide consumer information for outside resources and encourage independent access.
• Attend monthly staff meetings and meetings with other agencies/court as requested.
