Job summary

Applitrack is seeking a Case Manager at Chicago Heights Middle School to facilitate and coordinate special education meetings during case study evaluations and IEP meetings. The role reports to district administrators and involves MTSS participation and staff coordination.

The successful candidate will hold a current PEL with endorsement in School Psychology or LBS1, oversee multiple staffing processes, and ensure compliance with district and AFT Local 604 agreements.

Qualifications

• Current PEL with endorsement in School Psychology or LBS1 is required.
• Experience with MTSS meetings and case study evaluations is preferred.
• Familiarity with IEP processes and special education procedures.

Responsibilities

• Consult with staff and principals on referrals and MTSS needs.
• Receive, review, and process all special education referrals.
• Schedule and participate in initial case studies and IEP meetings.
• Coordinate data collection for case study staffing and evaluations.
• Inform staff and administrators of conferences at least one week in advance.
• Complete staffing papers and file appropriately.
• Review incoming student folders and maintain records.
• Coordinate out-of-district staffing with non-public schools.
• Maintain reevaluation lists and distribute data collection forms.
• Forward information to parents and external agencies when needed.
• Submit district case study staffing reports to the Special Education Director.
• Assist with preschool screenings and related diagnostics.
• Act as district liaison to the Director of Special Education.
• Coordinate and schedule Annual Reviews for assigned schools.
• Provide input beneficial for district programming and other duties.
