Gulf Bend Center seeks a Case Manager, School-Based Services to support children on school campuses and in the community, coordinating care with school staff and families.

The role emphasizes crisis intervention, care planning, and collaboration with partners to enhance access to services in rural areas and within the school setting. A BA/BS in a relevant field or RN license is required.

Qualifications

• Bachelor's degree in social/behavioral/human services or RN license required.
• Preferred: at least six months’ mental health service experience with adults, children, and adolescents.
• Social work, psychology, nursing, counseling, education psychology, or related fields may apply.

Responsabilités

• Provide School Based Case Management services per policy and contract.
• Deliver Crisis Intervention services per HHSC contract and center policy.
• Conduct case management, skills training, and medication training per Medicaid rules and guidelines.
• Complete CANS assessments per center and HHSC contract; formulate care plans with families.
• Provide 95% of services in consumers' natural environments (home, school, community).
• Submit all documentation within two business days.
• Track and manage caseload to maintain up-to-date consumer documentation.
• Maintain required fleet and safety procedures as per center directives.
