Job summary

Gulf Bend Center is seeking a Child and Adolescent Services Case Manager in Victoria, TX. The role provides case management, skills and medication training to children/adolescents with serious emotional disturbance, plus services to caregivers in school, community and clinic settings.

Required: BA/BS in a relevant field or RN license in Texas. Typical schedule is Monday–Friday, 8:00 am–5:00 pm, with travel as needed and a starting pay of $18.47 hourly for non-exempt staff.

Qualifications

• BA/BS in social, behavioral or human services or RN licensed in TX.
• Preferred: 6 months MH services experience with children/adolescents.

Responsibilities

• Completes TRR assessment within 7 days and keeps it current.
• Formulates Recovery Plan with consumer/family and TAC/HHS policy alignment.
• Provides authorized case management per Recovery Plan and Medicaid rules.
• Delivers 95% of services in the consumer's natural environment.
• Documents in electronic health record within 2 business days.
• Maintains caseload workflows and participates in team meetings.
