Job summary

The Case Manager 1 - School Based will provide strength-based services to middle school students and their families in North Central Massachusetts, meeting in schools and homes and coordinating care with community partners.

Ideal candidates hold a Bachelor's degree in a related field, have 3 years of experience with at-risk youth, and are bilingual/bicultural if possible. This full-time role offers travel within the community and a competitive benefits package.

Qualifications

• Bachelor’s degree in Human Services, Counseling, Social Work, Psychology or closely related field.
• 3 years of experience with at-risk youth and families.
• Bilingual/bicultural candidates encouraged to apply.

Responsibilities

• Meet with students/families at home or school regularly.
• Maintain communication with students and families between meetings.
• Complete intakes and discharges, including screening and assessments.
• Identify student/family needs and connect them to community resources.
• Model skills and mentor students and families.
• Facilitate psychoeducational groups for youth and families.
• Assist with program coordination and referrals within the school.
• Prepare internal reports and reports for the program funder.
