Job summary

Centerstone in Indianapolis is seeking a School Based Support Specialist to deliver intensive outpatient case management for youth in school settings. You will coordinate with schools, families, and providers, delivering individualized interventions aligned with treatment plans to promote recovery and academic success.

You will travel to homes and communities as needed, maintain timely documentation, and participate in ongoing staff development to adapt to student needs and program goals.

Qualifications

• Bachelor’s degree in psychology, social work, behavioral health, or related human services field, OR a four-year degree with two years related behavioral health experience.
• Associate’s degree in behavioral health support can qualify.
• Four years of full-time qualifying experience in the field may substitute for a degree.

Responsibilities

• Provide behavioral health interventions in school-based settings.
• Support coping strategies and daily living skills for students.
• Coordinate care with schools, families, and providers.
• Manage an assigned caseload with timely, person-centered documentation.
• Provide crisis intervention and outreach for missed appointments.
• Conduct home visits and family engagement outreach.
• Assist with quality assurance, data collection, and compliance with agency protocols.
• Participate in staff development and supervision activities.
• Adapt to evolving student needs and support program goals.
• Other duties as assigned.
