Job summary

Adult & Child Health in Indianapolis is seeking a School Based Support Specialist to deliver intensive outpatient case management for youth in a school setting, aligning interventions with treatment plans and coordinating care with schools and families.

You will conduct home visits as needed, maintain timely documentation, participate in staff development, and adapt to evolving student needs while promoting recovery, resilience, and academic achievement.

Qualifications

• Bachelor’s degree in psychology, social work, behavioral health, or related field or two years related behavioral health experience.
• Four-year combination of higher education and two years qualifying experience acceptable.
• Two years of related behavioral health experience if degree field different.

Responsibilities

• Provide school-based behavioral health interventions aligned with individualized treatment goals.
• Support students’ development of coping strategies and daily life skills.
• Coordinate care with schools, families, and community providers.
• Manage caseload, ensure timely documentation, and provide person-centered services.
• Provide crisis intervention and follow-up, including missed appointments outreach.
• Conduct home visits and outreach to engage youth and families.
• Support QA activities, data collection, and compliance with agency protocols.
• Participate in staff development, supervision, and evaluations.
• Adapt to evolving student needs and collaborate to meet program goals.
• Perform other duties as assigned.
