Job summary

Rosecrance Ware Center in Rockford, IL is seeking a dedicated behavioral health professional to support the Student Assistance Program in local schools. You will coordinate adolescent SAP services, provide trainings, assess needs, and manage casework within the core team framework, ensuring timely documentation and follow-up.

Qualifications

• Bachelor’s degree in psychology, counseling, social work or related field.
• Illinois CADC or IL licensed LSW/LPC within 2 years of employment in SUD.
• Must qualify as a MHP or higher per Illinois DMH Rule 132.
• Knowledge of DSM diagnoses and criteria.
• Experience with youth with mental illness or SUD preferred.
• Excellent written and verbal communication skills.
• Ability to establish rapport quickly.
• Computer proficiency required.
• Ability to complete documentation in the EHR within time frames.
• Valid driver’s license and own transportation.

Responsibilities

• Work hours prescribed; additional hours as needed.
• Coordinate SAP in the school setting with core team.
• Provide ongoing training with school personnel, students and parents.
• Assess behavioral health and SUD needs of students.
• Case management responsibilities for SAP participants.
• Close follow-up of current or former program participants.
• Track data for outcomes and program evaluation.
• Conduct group and individual support sessions at the school.
• Provide assessment services for referrals.
• Maintain clinical records per agency policies.
• Facilitate SAP team meetings and client coordination.
• Perform on-call procedures as designated.
• Assist in community PR activities and presentations.
• Develop a referral source list with core team.
• Serve on SAP and school core teams; participate in meetings.
• Comply with Rosecrance compliance and code of conduct.
• Deliver exceptional customer service to all customers.
• Serve as role model for staff and clients.
• Perform additional responsibilities as assigned.
