Job summary

CareGivers America - NEPA, a Modivcare Personal Care Service, seeks a Care Coordinator Associate to manage referrals, coordinate patient intake, and serve as the main point of contact for patients and providers. You will collaborate with teams to determine eligibility and ensure timely access to services.

The role emphasizes accurate record-keeping, scheduling, and compassionate support to patients and families in a community-based care environment.

Qualifications

• 1+ years of Customer Service experience required.
• Preferred bilingual English & Spanish abilities.

Responsibilities

• Receive and process referrals from hospitals, physicians and other providers.
• Collaborate with internal teams to determine patient eligibility and initiate intake.
• Serve as the primary contact for patients, families, and professionals regarding care coordination.
• Address inquiries and guidance on accessing services and resources.
• Schedule patient appointments coordinating provider availability and patient preferences.
• Monitor and update schedules for changes, cancellations and rescheduling.
• Communicate patient information including diagnosis and care plans to relevant parties.
• Maintain accurate electronic and physical records of patient information and communications.
• Provide compassionate and empathetic support throughout care coordination.

You will...

• Maintain accurate and up-to-date electronic and physical records of patient information, appointments, and communication.
• Provide compassionate and empathetic support to patients and their families throughout the care coordination process.

• High school diploma or equivalent
• 1+ years of Customer Service experience required.
• 1+ years of Home Careexperience preferred.
• Bilingual in English & Spanish preferred
