Job summary

Talentify is seeking compassionate Patient Care Coordinators to join our homecare team and help patients across Pennsylvania and Delaware access timely services.

You will screen patients, onboard families and caregivers, coordinate schedules, and ensure compliance, with comprehensive training provided to succeed in this role.

Qualifications

• Bilingual preferred.
• Excellent communication skills.
• Strong work ethic and dedication to helping others.
• No homecare experience required – training provided.

Responsibilities

• Screen new patients and caregivers.
• On-board new patients, including handling documentation/clearance.
• On-board new caregivers, including orientation/training.
• Staff patient cases with caregivers and initiate cases promptly.
• Ensure compliance with law and company policy (caregiver clock-in/out).
• Communicate with patients, caregivers, and families to ensure satisfaction.
• Assist with on-site visits as needed.

Responsibilities

• Screen new patients and caregivers
• On‐board new patients, including providing assistance with any documentation/clearance requirements
• On‐board new caregivers, including orientation/training
• Staff patient cases with caregivers and ensure patient cases are started promptly
• Ensure compliance with the law and Company policy, including caregiver clock‑in and clock‑out requirements
• Communicate with patients, caregivers, and patient families to ensure satisfaction and quality service delivery
• Assist with on‑site visits, as needed
