Renew Home Health is seeking a motivated Home Health Sales Representative to expand our referral network and connect patients with high-quality home health services. This role requires relationship-building with physicians, hospitals, and care facilities, plus travel within the assigned territory.

You will identify new opportunities, educate partners about services, and drive monthly growth while collaborating with clinical and administrative teams to ensure patient care and referral

Qualifications

• Strong communication, presentation, and relationship-building skills.

• Reliable transportation and willingness to travel throughout the assigned territory.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, case managers, rehabilitation facilities, assisted living communities, and other referral sources.
• Generate patient referrals for home health services.
• Educate healthcare professionals and community partners about our services and programs.
• Conduct regular in-person visits, presentations, and networking activities.
• Identify new business opportunities within the assigned territory.
• Track sales activities, referrals, and outcomes in the CRM system.
• Collaborate with clinical and administrative teams to ensure excellent patient care and referral satisfaction.
• Meet or exceed monthly growth goals.
