The Care Experience Specialist role focuses on presence, empathy, and listening. You’ll meet people where they live and gather, build trust, and help remove barriers to care, bridging the human and medical

Qualifications

• Associate degree or combination of education and experience required; B.S./B.A. preferred.
• 1–3 years of community-based outreach/health education experience.
• Valid driver's license and reliable transportation preferred.
• Background check and training prior to field deployment.
• Bilingual or multilingual abilities (e.g., Spanish, Haitian Creole, Russian) a plus.

Responsibilities

• Engage Family Members where they live, worship, and gather.
• Conduct in-person and phone outreach with high- or rising-risk Family Members.
• Build trust through consistent, culturally aligned communication.
• Surface and address barriers to care such as food insecurity, transportation, housing.
• Assist with paperwork for social services and basic needs.
• Support in-home technology setup to enable virtual visits.
• Provide warm handoffs and referrals to care team and partners.
• Document interactions and engagement in CINQIQ and track trust.
• Represent CINQCARE at community events and maintain local trusted contacts.
